This questionnaire takes about 15 minutes to complete.
You can answer the questions online or request a printable copy of the questionnaire and email us your responses. You can also attach documents, or audio or video files if you want to at the end of the form.

About Foundational Supports
Governments are working together to design and deliver additional supports in the community. These are known as Foundational Supports.
They are specific supports that are additional to mainstream services and supports accessed through the National Disability Insurance Scheme (NDIS).
There are different types of foundational supports. This questionnaire asks questions about Foundational Supports for children with developmental concern, delay and/or disability and their families, carers and kin.
Foundational supports for children include General Supports as well as Targeted Supports. Targeted supports would be for children with developmental delay and their families. These children would not be in the NDIS and need more assistance than mainstream services and General Foundational Supports.
What is this questionnaire about
The questions are mainly for families, carers and kin of children aged under 9 who have developmental concern, delay and/or disability. However, organisations and other interested individuals can also take part in the questionnaire or skip the questions to upload a submission.
We will ask about:
- How you get information and advice about support you or your child might need and how to access it?
- How you can be supported to connect with and navigate services, make decisions, and take part in the community?
- What the barriers are to getting support for your child or family?
- What additional supports you need and what that might look like?
- How supports outside of the NDIS could work better together including with mainstream settings like childcare?
When we say ‘your child’ we are referring to a child or children that you are a parent or guardian of or who you may care for.
We suggest you refer to the Consultation Paper when you are completing this questionnaire as this provides more detailed information about what these supports might include. Some information from the paper is included in the introduction to the questions.

If you respond as an organisation, you may provide the name of the organisation.
